Cardiovascular disease and cardiac conditions
If you have a history of heart disease, recent myocardial infarction, unstable angina, arrhythmias, decompensated heart failure, severe aortic stenosis, or any other cardiac condition, consult your cardiologist or primary care physician before starting any sauna practice. While observational evidence on sauna and stable cardiac conditions is generally favorable, heat exposure carries real risks in unstable or severe cardiac disease, and initiation should be supervised or cleared by a clinician who knows your history.
Hypertension
Sauna produces significant cardiovascular and hemodynamic changes. If you have uncontrolled or recently diagnosed hypertension, or if your blood pressure medications have been changed recently, speak with your prescribing physician before using a sauna regularly.
Pregnancy
The safety of regular sauna use during pregnancy, particularly in the first trimester, is not established. Some evidence associates significant core temperature elevation in early pregnancy with increased risk of neural tube defects and other fetal outcomes. Do not make decisions about sauna use during pregnancy based on this site. Consult your obstetrician or midwife directly.
Medications
Many medications affect thermoregulation, fluid balance, cardiovascular response, or heat tolerance. These include but are not limited to: diuretics, antihypertensives (especially nitrates, alpha-blockers, ACE inhibitors), beta-blockers, anticholinergics, tricyclic antidepressants, antipsychotics, stimulants including prescribed ADHD medications, and lithium. Alcohol is a strict contraindication around a sauna session. If you take any of these, discuss sauna use with your prescribing clinician before starting a regular practice.
